If you’re a third year medical student, you know how frightening patient presentations can be. When you’re just starting off, you NEED a template to wrap your head around patient presentations. In the past, I would type up a running document and edit it every morning before rounds. However, quickly you’ll realize that getting to work 1 and a half hours before rounds is not sustainable and people will start to think you’re crazy.
Then, I started printing out a bunch of H&P and Progress Note templates. Each morning, I would identify the key information on my patients and present the daily events in SOAP note format. This worked out great. However, once my patient load started increasing, patients started to blur and days got mixed up as well. Often, I would be asked, “What was his white count yesterday” or “When did he start having diarrhea?”. I would try to scramble through my many sheets of progress note templates in my white coat pocket. Far too often, this would never work. And by the time I could find that information, we had already moved on to the next patient.
